A state investigation results in a fine for a KUB contractor after a fatal accident.

27-year-old Michael Tallent of Powell was electrocuted on New Year's Eve, at KUB's wastewater treatment plant on Neyland Drive.  He worked for W and O Construction Company, Incorporated.

Investigators say Tallent was holding stabilizing cables attached to a crane when the crane hit overhead power lines.

An investigation by the Tennessee Division of Occupational Safety and Health (TOSHA) found several serious violations.

A report notes that the company didn't have the power cut to the electrical lines and didn't train employees properly on grounding equipment and handling power lines.

TOSHA fined the company $24,000 dollars for the violations in April.
The payment is due tomorrow.
